Your problem points to the need for planning and organizing your routine. It could also be because you have been used to one routine. Now, the need to balance or alternate between home and work is overwhelming.

Plan your day: There is a fixed routine at home and work—plan both. Segregate the time and factor in the changes you need to make in the schedule.
Seek help: If you want someone to help you with things at home- chores and for help with childcare. Have a conversation with everyone concerned to get support.
Talk to your leader: If there is a scheduling or a time constraint that crops up time and again, you might want to involve your leader. Discuss how you can make your schedule flexible, as today, most organizations are keen on flexible timings. It is one of the benefits brought by Covid.
Stick to your schedule: Difficult as it may sound; it is not impossible. You are always in control of the events when you go by a schedule.
Factor in uncertainties: When following a plan, you might also have a sudden event or incident derailing it for the day and can cause a panic. Have contingencies in the back of your mind and consider them.
When you juggle work and home with a clear plan and means to be effective on both fronts, you will always find success.
